Health New Zealand | Te Whatu Ora is seeking a Nurse Practitioner to work across primary care practices and Community Mental Health teams. About the Role
Theprimary purpose of the role is to workindependently and collaboratively with the primary care practices across thespecific localities and the locality Community Mental Health teams as an autonomous practitioner. The Mātanga TapuhiNursePractitioner (NP) role is integral to providing anindividual and population-based approach to meet mental healthcare needs.This community basedrole improves access to mental health care provision for patients acrossthe primary and tertiary care settings. This requires aflexible approach, to provide consultation, direct care and treatment enablingepisodes of care management, as well as support through clinical advice andeducation. The advanced practice role is integralto the provision and ongoing development of high-quality tangata whaiora care - that promotesoptimal experience and equitable outcomes for tangata whaiora and their whānau.
About theTeam/Service/ Location
MHAIDS spans two districts - Wairarapa, Capital, Coast andHutt Valley and includes local, regional, and national services. The localMHAID services are provided from multiple sites within the sub-region - greaterWellington, Hutt Valley and Wairarapa. Te Korowai Whāriki services includeregional forensic and rehabilitation services covering the Central region whilethe intellectual disability services extend the length and breadth of thecountry from six bases located in Whangarei, Auckland, Cambridge, Wellington,Christchurch, and Dunedin. The core business of the Service is the provision ofquality specialist services, integrated with primary and secondary providers inthe communities we serve.
The Mental Health, Addiction and Intellectual DisabilityService (MHAIDs) has created a Primary Care Liaison (PCL) Service to strengthenthe interface between primary care and secondary specialist mental health care.The long-term direction of the PCL service is to improve mental health servicedelivery within primary care settings and reduce demand for secondary services.The PCL team supports communication between services, support early specialistmental health service intervention, and offer timely clinical advice to primarycare clinicians.
