Job summary

Health New Zealand | Te Whatu Ora is seeking a Needs Assessor Mental Health in Hawke's Bay to support people with mental health and addiction needs to live independently and with improved wellbeing. You will work with tangata whaiora, whānau, and teams to assess needs, develop plans and coordinate services in the community.

The role emphasises equitable outcomes for Māori and diverse communities, with a focus on person-centred care and building strong partnerships across health and community

Qualifications

• Registration with a New Zealand professional body and an Annual Practising Certificate.
• Experience in mental health, addictions, nursing, social work, occupational therapy, or a related health profession.
• Strong communication and relationship-building skills across services.
• Commitment to culturally safe practice and achieving equitable outcomes for Māori and diverse communities.
• Excellent assessment, care coordination, problem solving, and time management skills.

Responsibilities

• Undertake comprehensive needs assessments with clients.
• Develop personalised support plans to meet goals.
• Coordinate access to services to improve wellbeing.
• Collaborate with tangata whaiora, whānau, community providers and multidisciplinary teams.
• Promote equitable access to support within the community.
