An exciting opportunity for an experienced, registered Mental Health Nurse to join our friendly and dynamic Fresh Minds team in Manukau.
This liaison role involves working with primary care providers in Counties Manukau to promote the Wellness Support model of care and support new practices that join ProCare to understand primary mental health pathways, systems, and processes in place to support their patients. It involves working with GP practices and a range of primary care health professionals to improve their knowledge of mental health and addiction issues through presentations and case consultations, and to enhance their skills in identifying and addressing them effectively.
The Mental Health & Addictions Coordinator plays a role in coordinating the South Auckland Special Interest Group (SASI) sessions alongside other PHO Coordinators and providing mentoring and/or supervision to interested health professionals (including credentialed nurses and those engaged in Primary Mental Health Education).
