Job summary

Health New Zealand | Te Whatu Ora is seeking a Kaiāwhina to support whānau living with chronic conditions, focusing on renal and diabetes services in Hastings. You will provide cultural support, care coordination and community engagement to help people access needed care.

You will work with patients, whānau and clinicians to remove barriers to care, reduce missed appointments and empower communities to manage health, contributing to equitable healthcare across Hawke’s Bay.

Qualifications

• Experience in health sector, community services or similar people-focused environment.
• Strong communication and relationship-building with patients, whānau and community partners.
• High level of cultural awareness, especially regarding Māori and Pacific communities.
• Excellent organisational skills with ability to manage priorities and maintain records.
• Full New Zealand driver’s licence and ability to work in a multidisciplinary team.

About the Role

As a Kaiāwhina, you will play a key role in supporting whānau living with long term conditions, with a particular focus on Renal and Diabetes services. Working closely with patients, whānau and clinical teams, you will provide cultural support, patient advocacy, care coordination and community engagement to help people access the care and support they need.

About the Team/Service/ Location

Our Long Term Conditions team is part of the Specialist Community Services Group and is dedicated to improving the health and wellbeing of people living with chronic health conditions. We work collaboratively with patients, whānau, primary care providers, community organisations and Māori health services to deliver person centred and culturally responsive care.

Based in Hastings, you will join a supportive team that is passionate about improving health equity and helping people achieve the best possible health outcomes within their communities.

About you

As a Kaiāwhina, you will bring:

• Experience working within the health sector, community services, education, or a similar people focused environment.
• Strong communication and relationship building skills, with the ability to engage effectively with patients, whānau and community partners.
• A high level of cultural awareness, particularly regarding Māori and Pacific communities.
• Excellent organisational skills, including the ability to manage competing priorities and maintain accurate records.
• A full New Zealand driver's licence and the ability to work both independently and as part of a multidisciplinary team.
