Job summary

Qualifications

• NZ Registered Social Worker with current Annual Practicing Certificate.
• Minimum of 3 years clinical practice essential.
• Experience in health settings preferred.

Responsibilities

• Support clients and whānau through health journeys in hospital and community.
• Collaborate with interdisciplinary teams including Allied Health and Community Nursing.
• Provide social work assessment, planning and discharge support.
