As a Health Improvement Practitioner at Te Kōhao Health, you will:
Work within a general practice setting to deliver evidence-based behavioural interventions for individuals, groups, and whānau.
Support clients' needs and transitions between different healthcare services.
Collaborate with a multidisciplinary team to provide comprehensive care.
About You:To excel in this role, you will possess:
An annual practising certificate as a registered health professional.
Clinical experience in mental health settings, providing evidence-based psychological interventions.
Knowledge of behavioural medicine and the medical-psychological relationship.
Demonstrated knowledge of Te Reo me ōna tikanga.
Effective relationship management skills and an understanding of the healthcare system.
The ability to connect with people from diverse backgrounds and cultures.
Excellent communication, organisational, and planning skills.
