Job summary

Crossing Hurdles in New Zealand seeks experienced medical records reviewers to support AI training projects. You will review records, examine FHIR data and clinical notes, and verify that information and claims are supported by the medical record.

You will assess inpatient documentation (HPI, PMH, assessments, plans), identify gaps or inconsistencies, and provide actionable feedback to help improve data quality. Strong communication with the project team is essential.

Qualifications

• MD or DO with board certification in Family Medicine.
• Strong experience reviewing inpatient documentation and clinical notes.
• Comfortable working with raw medical record data and notes.
• Experience with chart review or medical coding is a plus.
• Ability to communicate findings clearly to project teams.

Requirements

• Have strong relevant experience in the domain.
• MD or DO, board certified in Family Medicine (or Family Medicine with a Hospital Medicine focus or certification).
• Strong understanding of inpatient documentation and hospital workflows, including admission, daily progress notes, transitions of care, and discharge.
• Comfortable reviewing clinical notes and raw medical record data.
• Experience with chart review, clinical documentation improvement, utilization review, or medical coding/QA is a plus.
