Job description

Te Hau Ora O Ngapuhi is an organisation with a mission to provide health and wellbeing to the people of Northland, with a focus on Maori in particular. Our services are whanau centred and we are committed to making a difference in improving the social and wellbeing outcomes for whanau. We are focussed on making sure our community is informed and supported as much as possible.

As the Kaimanaaki - Mate Wareware (Dementia) - Whangarei you will be able to support whanau living with disabilities and their whānau, providing Whaikaha support through a Māori Iwi Needs Assessment & Co-ordination Service.

Supporting whānau to live their best possible lives.

  • Supporting tāngata mate wareware, their whānau and carers to understand health literacy of available best management options
  • Coordinating access to appropriate screening, diagnostic assessments, and appropriate education in the community
  • Coordinating appropriate services for Tāngata Mate Wareware, their whānau and carers in non-traditional health care settings
  • Building and maintaining relationships with providers to support whānau ora outcomes and holistic wellbeing
  • Ensuring appropriate educational resources are available for Tāngata Mate Wareware, their whānau and carers
  • Identify needs and support people to access to relevant health focussed activities
  • Link with established Dementia Mate Wareware groups and providers in Te Tai Tokerau to ensure Tāngata Mate Wareware, their whānau and carers receive the support and resources they need.
  • Promote Dementia Awareness month across the community and supporting with local activities.
  • Working collaboratively with the specialist Dementia and Alzheimers NGO across the community / region to promote best practice, collaboration, integrated care and improved health outcomes for Tāngata Mate Wareware, their whānau and carers.
  • Ensuring access to appropriate clinical support as needed, based on continuous improvement of services i.e., including education programmes.
  • Create a digital wallet for Tāngata Mate Wareware, their whānau and carers so they are aware of local services and programmes.
  • Work with whānau to develop their capacity to maintain healthy lifestyles, and support behaviour change where appropriate to improve their health and wellbeing.
  • Access to cognitive stimulation therapy and other evidence based interventions to maximise function for Tāngata Mate Wareware.

As the successful candidate you will have the following:

  • Clean full New Zealand Drivers Licence
  • Information technology and administration literacy
  • Commitment to working effectively with Maori, Pacific, Culturally and Linguistically Diverse (CALD) communities
  • Maori cultural competence and an understanding of tikanga Māori
  • Community awareness and an ability to engage whānau
  • The ability to carry out wellbeing checks
  • Understand confidentiality expectations with regards to entering homes, engaging with whānau, collecting and recording data
  • Meet all standard employment prerequisites
  • Excellent communication and interpersonal skills
  • Ability to establish rapport with children and whānau
  • Ability to set and maintain professional boundaries
  • Excellent time management and organisational skills
  • Able to work collaboratively in a range of formal and informal team setting and autonomously as the situation requires
  • Commitment to holistic, flexible and responsive working
  • Willingness to work with whānau in their homes
  • Willingness to work in a multidisciplinary setting and across different agencies
  • Ability to engage and maintain relationships
