Health New Zealand | Te Whatu Ora Lakes in Rotorua is seeking a District Nurse to cover parental leave on a 0.6 FTE basis. The role involves assessing, planning and delivering care in partnership with patients and whānau across home and community settings.

You will work with a multidisciplinary team to provide holistic care, support self-management and facilitate transitions between hospital and community care. A current NZ Nursing Council registration and PDRP Level 3 are required.

Qualifications

• Holds New Zealand Nursing Council registration (NCNZ).
• Holds a current Practicing Certificate.
• Holds a current drivers licence for both manual and automatic vehicles.
• Have evidence of practicing at Level 3 of an approved PDRP programme.
• Have at least 4-5 years post graduate nursing experience in an acute medical/surgical setting.
• Possess Advanced IV management (CVAD), Port-A-Cath, PICC, Midline experience.

Responsabilités

• Assess, plan and implement programmes of care with patients, family and whānau.
• Conduct daily home visits, start early, perform home-based treatments, and coordinate with the multidisciplinary team.
• Document care and communicate with the wider healthcare team to provide holistic care.
• Contribute to transitional care between hospital and community.
