Total Care Health Service is seeking a Clinical Coordination Lead to provide clinical expertise and high-quality coordination for community nursing services across Aotearoa.

We are seeking a Clinical Coordination Lead to provide clinical expertise and high-quality, client-centered coordination for community nursing services across Aotearoa. This role supports safe and timely entry to service, clinical triage, allocation support, and coordination through to discharge.You will work closely with Clinical Nurse Leaders, Community Nurses, the National Referral Centre, administration teams, referrers and funders to support safe service delivery, quality improvement and positive outcomes for tāngata and whānau.
We have part-time and full-time opportunities available.
