Job summary

Aberdeen City Hospital at Home Service is seeking an experienced Advanced Practitioner to deliver high-quality acute care in patients' homes. You will assess, diagnose, treat and manage complex conditions autonomously within a rapidly expanding hospital-at-home model.

The role requires a Master’s level education in Advanced Clinical Practice and a V300 Independent Prescriber, with leadership and teaching responsibilities across a multidisciplinary team.

Qualifications

• Postgraduate Diploma (Master's level) in Advanced Clinical Practice or expected to complete within six months.
• Credentialed with Master's level education and substantial clinical experience in one or more Advanced Practice roles.
• Independent Prescriber (V300) with autonomous prescribing experience in advanced practice settings.
• Significant post‑registration clinical experience across Acute Medicine, Frailty or Respiratory Care.
• Leadership, service development, teaching, mentorship and CPD are essential.

• Assess, diagnose, treat and manage patients with acute and complex needs at home.
• Deliver safe, effective, person-centred care and contribute to service development and innovation.
• Lead teaching, mentorship and clinical supervision within the team.
