Job summary

Greater Lawrence Family Health Center is seeking a Behavioral Health Liaison to join our Behavioral Health Program. The BHL serves as a key link between patients referred by the PCP to Behavioral Health Services, facilitating warm handoffs, scheduling, and tracking referrals to improve access to care.

Responsibilities include processing referrals, verifying insurance, authorizations, coordinating with BH specialists, and ensuring follow-up on non-clinical issues.

Qualifications

• Certificate in Human Services or related field.
• 3+ years' direct patient care experience.
• Bilingual Spanish and English.
• Strong interpersonal and communication skills.
• Cultural competence and ability to work with diverse populations.

Responsibilities

• Serve as liaison between patients, PCPs and Behavioral Health Services.
• Assist patients referred to mental health services as advocate and navigator.
• Process referrals, prior authorizations, scheduling and tracking of BH referrals.
• Review health insurance status and obtain prior authorizations as needed.
• Coordinate with BH specialists to develop comprehensive care plans.
• Provide follow-up from visits or calls for non-clinical issues.
