Job summary

Together for Youth seeks a Prevention Family Specialist to provide in-home casework counseling and empower parents with tools and resources for children's safety, well-being, and permanency. The role includes advocacy to ensure quality services.

Responsibilities include home visits, coordination with schools and community resources, and on-call duties in rotation with the program. A Bachelor's in human services is required, with at least 1 year in at-risk youth work.

Qualifications

• Bachelor's degree in human services or related field.
• Minimum of 1 year of experience working with at-risk youth & families and case management.
• Must be able to work a flexible schedule with variable hours including day, evening, and on-call hours.

Responsibilities

• Conducts assessment of family strengths and needs and uses treatments and resources to promote growth and change.
• Develops and evaluates comprehensive treatment and discharge plans; identifies permanency resources.
• Delivers case management to ensure counseling sessions, medical/dental exams, and timely documentation; maintains confidentiality.
