Job description

Overview

Adherence to nursing care vision, mission, goals and objectives; aligns with the scope of nursing services and performs shift duties as assigned by the Unit Manager to ensure high quality care. Notifies the unit manager of changes that could impact care standards.

Responsibilities

• Obtain and hand over a report of patient progress, ensure all doctor’s orders are followed before changing or taking over shifts, and notify of incidents or problems.
• Visit patients with the team to assess conditions, re-plan nursing care, and determine nursing priorities based on patient needs and staff availability.
• Provide comprehensive hemodialysis patient care using the nursing process and devices (hemodialysis machines, CRRT, various access sites); manage all phases of hemodialysis treatment; maintain patient records; promptly report abnormalities to medical professionals.
• Conduct nursing rounds, provide emotional support, accompany doctors during rounds, assist in procedures as needed.
• Execute doctor’s orders, coordinate with the team on patients’ treatment, and foster effective communication with all disciplines, patients, and relatives to ensure patient satisfaction.
• Engage in health education for patients and families; participate in unit meetings and hospital activities; advise on organ donation programs and plan treatment programs for adequate care.
• Provide clear, compassionate, and culturally sensitive communication about condition, treatment options, and care plan.
• Assist with patient billing procedures, schedule next appointments, and guide patients to the cashier for payment processing after treatment.
• Collaborate with the unit manager in administration, set care standards, and fulfill duties in the absence of the unit manager; perform assigned tasks and provide feedback.
• Charge accurately, including equipment and consumable items, and verify completeness of patient charges.
• Inspect and document sterile pack supply to ensure packaging integrity; maintain adequate unit stock from CSSS.

Human Resources and Development

• Maintain personal and professional growth; keep up to date with mandatory learning hours; participate in continuous professional development through annual competency evaluations and every 3 years’ privileges; proficient in cannulations and medication calculations.
• Participate in teaching programs and orient new staff to ward routines.
• Supervise staff including Assistant Nurses, Midwives, Care Assistants, and student nurses to achieve clinical objectives.
• Collaborate with the unit manager to evaluate staff performance, optimize resource use, and coordinate with consultants for bed management.
• Contribute to development and maintenance of the nursing process, ensure effective patient care and documentation; participate in research initiatives and quality improvement through evidence-based practice.

Policies and Procedures

• Participate in review and revision of nursing policies, procedures, and work instructions to promote best practices and continuous improvement.
• Operate with proficiency in implementing unit work instructions, policies and procedures for optimal treatment of dialysis patients.
• Inspect DDA drugs with the previous shift’s nurse in charge and adhere to hospital policies on medication care, custody, and administration.
• Check daily and audit resuscitation trolley as scheduled; promote infection prevention and control by ensuring staff follow protocols and monitoring patients with appropriate care bundles; maintain confidentiality of patient information.

Facilities and Equipment

• Operate and maintain specialized equipment; guide new staff on use and maintain accurate records.
• Perform pre-use checks on vital equipment (e.g., ventilators) for safe functioning.
• Collaborate with the unit manager to maintain equipment, supplies, and drugs in line with the PPM schedule; notify biomedical personnel of faults and assist in cleaning, disinfection and proper handling.
• Participate in quarterly stock counts with finance staff.

Safety and Quality Improvement

• Participate in activities for continuous improvement of nursing services, including quality assurance, infection control, and fire and safety programs.
• Conduct thorough investigations, root cause analyses, and timely incident reporting; collaborate with the unit manager to collect and evaluate clinical and administrative data.
• Participate in hospital-wide surveys (e.g., MSQH, JCI, IMS) for quality improvement; contribute to Baby Friendly Hospital Initiative focusing on exclusive breastfeeding.
• Adhere to applicable safety regulations and guidelines, including occupational safety and patient rights policies.

Education, Skills and Attributes

• Education: Registered with the Nursing Board of Malaysia with a valid license to practice as a Registered Nurse.
• Skills: BLS certification; proficient in English and Bahasa Malaysia; Microsoft Office literacy.
• Attributes: Strong interpersonal and communication skills; critical thinking, problem-solving, caring, honesty, and responsibility; ability to work shifts, weekends, holidays, and emergencies; commitment to patient-centered care and continuous quality improvement; high integrity, patience, and attention to detail.
