What You’ll Do:

• Visit clients in their homes to assess mobility, balance, and safety

• Develop and implement individualized care plans and evidence-based interventions

• Provide education to clients and families to support independence

• Collaborate with a multidisciplinary care team

• Document care electronically using a state-of-the-art tablet

What You’ll Need:

• Active Delaware PT license

• Graduation from an accredited PT program

• Ability to work independently, with strong communication and time management skills
