General Description
A licensed healthcare professional authorized by law to dispense medications prescribed by consultants or other registered medical practitioners. The pharmacist coordinates and oversees all pharmacy-related activities to ensure accurate, safe, and effective medication management. Works collaboratively with other medical personnel to ensure optimal patient care through appropriate drug therapy and education.
Job Description
- Deliver pharmaceutical care in alignment with the vision, mission, core values, and goals of the Pharmacy Services and the hospital.
- Performs, assists and supervises all activities in the pharmacy including dispensing, sales and supply/ dispensing of medications.
- Contributes to in-service training for healthcare employee and provide counseling on the use and care of prescription drugs to patients.
- Ensures that dangerous drugs and psychotropic drugs are handled, stored and issued according to laws and regulations and hospital policies.
- Checks and endorses all outpatient prescription and TTA prescription (prescription intervention).
- Monitors drug therapies for drug interactions, allergies and contraindications.
- Monitoring and controlling inventory to ensure that drugs are available when needed and wastage is minimized.
- Checking of chemotherapy orders and cytotoxic preparations prior to cytotoxic drug reconstitution and prior to dispensing to the designated wards.
- Handles inquiries concerning drug contents, dosages, regimen review errors, storage and disposal.
- Involves in medication reconciliation process by validating patient’s own medications.
- Ensures and advises on safe disposal of pharmaceutical products.
- Involved in identifying lapses/challenges in the services and contributes to solving and decision making and to conduct root cause analysis in case of medication error.
- Reports and documents adverse drug reactions and medication errors.
- Involved in drug inventory procedures, inventory management, purchasing, receiving, drug supplies and medication storage of the services.
- To uphold best practices by using 7R rules - Right drug, right patient, right frequency, right duration, right route, right information and right documentation.
Education
- Bachelor’s degree in Pharmacy.
- Registered with the Malaysian Board of Pharmacy.
- Possess Poison License A.
Knowledge And Experiences
- Experience working in a hospital setting is an added advantage.
