Hospital Pharmacist

Prescriptions are screened and reviewed to comply with Poison Act.

Responsible for all aspects of quality and quality control of extemporaneously prepared drugs.

All dispense medicines are labelled according to the law.

Maintain effective daily dose system for wards.

Supervise repackaging and dilution of preparations by Pharmacy Assistants and Pharmacy Technicians.

Ensure that accurate records of all psychotropic substances and narcotic drugs are maintained in the Pharmacy department.

Using 5Rs when dispense medicine.

Educate and counsel patients pertaining to drug therapy and drug related side effect preventions/precautions.

Monitor and report adverse drug reactions, prescribing errors, order entry errors, medication errors and packing errors.

Supervise Technicians and Dispensers on their respective duties.

To check on storage and maintenance of medication in other units according to hospital policy.

Attend regular meetings with departmental staff and other relevant staff to ensure smooth operation and implementation of the departmental objectives and work together to achieve and maintain the hospital’s accreditation.

To act as link ward pharmacist.

To review the antimicrobial resistance prescriptions.

Participate in quality assurance activities to ensure the maintenance of standards within the pharmacy division.

Perform other duties as assigned by the Chief Pharmacist.

REQUIREMENTS AND QUALIFICATION

Minimum Bachelor's Degree in Pharmacy (recognized by Malaysian Pharmacy Board).

Registered with Malaysian Pharmacy Board with valid Pharmacy license.

At least 2 years of working experience in hospital setting.

Willing to work in nuclear pharmacy section and undergo the appropriate training.
