Job Summary: The Consultant Gastroenterologist is responsible for providing expert medical care to patients with a wide range of disorders affecting the digestive system and associated organs (liver, gallbladder, pancreas). This involves comprehensive patient evaluation, accurate diagnosis, formulation and execution of evidence-based treatment plans, performance of various endoscopic procedures, and collaborative work within a multidisciplinary healthcare team to ensure optimal patient outcomes, all in accordance with Malaysian healthcare standards and regulations.
Key Responsibilities:
Patient Consultation and Diagnosis:
Conduct thorough medical histories and physical examinations for patients presenting with gastrointestinal (GI) and hepatobiliary symptoms.
Order, interpret, and evaluate a variety of diagnostic tests, including:
Blood tests (e.g., full blood count, liver function tests, pancreatic enzymes, inflammatory markers, serology for infections, celiac disease markers).
Stool analysis for infections, occult blood, or malabsorption.
Breath tests (e.g., for H. pylori or lactose intolerance).
Advanced imaging studies such as ultrasound, CT, MRI, and specialized GI imaging (e.g., enterography, colonography).
Endoscopic procedures (detailed below).
Diagnose and manage a broad spectrum of conditions affecting the digestive tract, including but not limited to:
Oesophageal disorders (e.g., GERD, oesophagitis, Barrett's oesophagus, dysphagia).
Gastric and duodenal disorders (e.g., peptic ulcer disease, gastritis, H. pylori infection, dyspepsia).
Small bowel diseases (e.g., celiac disease, malabsorption syndromes, Crohn's disease).
Colonic and rectal disorders (e.g., Inflammatory Bowel Disease - Crohn's disease and Ulcerative Colitis, Irritable Bowel Syndrome - IBS, diverticular disease, colonic polyps, colorectal cancer, constipation, diarrhea).
Pancreatic disorders (e.g., pancreatitis - acute and chronic, pancreatic cysts, pancreatic cancer).
Biliary tract disorders (e.g., gallstones, cholangitis, strictures, bile duct cancers).
Liver diseases (e.g., viral hepatitis A, B, C, fatty liver disease - NAFLD/NASH, MASLD/MASH, cirrhosis, liver cancer, autoimmune liver diseases, genetic liver disorders).
Gastrointestinal bleeding from various sources.
Treatment Planning and Management:
Develop and implement individualized, evidence-based treatment plans, incorporating pharmacological therapies, lifestyle modifications, and dietary advice.
Prescribe and manage medications, including proton pump inhibitors, immunosuppressants, biologics for IBD, antiviral agents for hepatitis, etc.
Provide long-term management for chronic GI conditions, including regular monitoring and adjustments to treatment.
Educate patients and their families about their conditions, treatment options, prognosis, and preventive measures, often requiring proficiency in Bahasa Malaysia and/or other common local languages.
Specialized Procedures:
Perform and interpret a wide range of diagnostic and therapeutic endoscopic procedures, including:
Gastroscopy (OGDS - Oesophago-Gastro-Duodenoscopy): Examination of the oesophagus, stomach, and duodenum, with biopsies and therapeutic interventions (e.g., polypectomy, haemostasis, foreign body removal, dilation).
Colonoscopy: Examination of the colon and rectum, with biopsies and therapeutic interventions (e.g., polypectomy, haemostasis, stricture dilation).
Endoscopic Retrograde Cholangiopancreatography (ERCP): Diagnostic and therapeutic procedure for bile duct and pancreatic duct disorders (e.g., stone removal, stent placement, stricture dilation).
Endoscopic Ultrasound (EUS): Advanced imaging combined with endoscopy for detailed visualization and biopsy of the GI tract wall and surrounding organs (e.g., staging GI cancers, evaluating pancreatic and biliary lesions).
Capsule Endoscopy: Non-invasive visualization of the small intestine.
Liver Biopsy: Percutaneous or transjugular liver tissue sampling for diagnosis of liver diseases.
Manage complications related to endoscopic procedures.
Interdisciplinary Collaboration:
Work closely with a multidisciplinary team including surgeons (general, colorectal, hepatobiliary), radiologists, pathologists, dietitians, nurses, and other allied health professionals.

This role involves accurate diagnosis, development and implementation of advanced treatment strategies, performance of specialized procedures, and active collaboration within a multidisciplinary team to achieve optimal patient outcomes in adherence to Malaysian healthcare standards.
Key Responsibilities:
Clinical Consultation and Diagnosis:
Conduct thorough patient histories, physical examinations, and detailed evaluations.
Order, interpret, and evaluate relevant diagnostic tests, including but not limited to:
Liver function tests, viral hepatitis serology (Hepatitis A, B, C, D, E), autoimmune markers, and other specialized blood tests.
Advanced imaging modalities such as ultrasound, CT, MRI, and specialized liver imaging (e.g., FibroScan/elastography for liver stiffness assessment, commonly used in Malaysia for fatty liver and fibrosis assessment).
Perform and interpret liver biopsies (percutaneous, transjugular).
Diagnose and manage the spectrum of liver conditions prevalent in Malaysia, including:
High prevalence of viral hepatitis (especially Hepatitis B and C).
Non-alcoholic fatty liver disease (NAFLD/NASH), which is increasingly common.
Cirrhosis and its complications (portal hypertension, ascites, hepatic encephalopathy, variceal bleeding).
Autoimmune liver diseases.
Genetic and metabolic liver disorders.
Drug-induced liver injury, including from traditional or complementary medicines.
Hepatocellular Carcinoma (HCC) and other liver cancers.
Gallbladder and bile duct pathologies (e.g., gallstones, cholangitis).
Treatment Planning and Management:
Formulate and execute individualized treatment plans, adhering to the latest evidence-based guidelines and national clinical practice guidelines where applicable.
Prescribe and monitor medications, including antiviral therapies for hepatitis, immunosuppressants, and supportive care for liver failure.
Manage acute liver conditions, liver emergencies, and chronic disease progression.
Provide pre- and post-liver transplant evaluation and management (if specializing in transplant hepatology, often at designated national transplant centers).
Educate patients and their families effectively, often requiring proficiency in Bahasa Malaysia and/or other common local languages, on disease management, dietary and lifestyle modifications, and prognosis.
Procedures (as credentialed and required by the institution):
Perform and interpret liver biopsies.
Conduct advanced endoscopic procedures such as Endoscopic Retrograde Cholangiopancreatography (ERCP) and Endoscopic Ultrasound (EUS) for biliary and pancreatic diagnosis and intervention.
Perform endoscopic management of varices (banding, sclerotherapy).
Perform therapeutic paracentesis for ascites.
(Less commonly performed by hepatologists, but may collaborate on or supervise for advanced interventional procedures like TIPS, RFA, TACE, etc., typically done by interventional radiologists or surgeons).
Interdisciplinary and Multispecialty Collaboration:
Actively participate in multidisciplinary team meetings (e.g., liver tumor board, transplant listing meetings) with local colleagues including gastroenterologists, liver surgeons, interventional radiologists, oncologists, pathologists, dietitians, and nurses.
Liaise with primary care physicians and other specialists for seamless patient referral and co-management within the Malaysian healthcare network.
Research, Education, and Quality Improvement:
Engage in Continuous Professional Development (CPD) activities to maintain and enhance medical knowledge and skills as required by the Malaysian Medical Council (MMC).
Participate in clinical audits, quality improvement projects, and contribute to local or national registries (e.g., Malaysian Liver Disease Registry).
Where applicable, contribute to teaching and training of medical students, house officers, medical officers, and gastroenterology/hepatology trainees.
Participate in clinical research or trials, particularly those relevant to endemic liver diseases in the region.
Documentation and Professional Standards:
Maintain meticulous and comprehensive patient medical records in accordance with Malaysian healthcare regulations and hospital policies.
Adhere strictly to medical ethics, patient confidentiality, and professional conduct as stipulated by the Malaysian Medical Council (MMC) and other relevant bodies.
Ensure compliance with all hospital and Ministry of Health (MOH) policies and guidelines.
Qualifications and Requirements (Malaysian Specific):
Medical Degree: MBBS or MD from a university recognized by the Malaysian Medical Council (MMC).
Full Registration: Possess current and valid full registration with the Malaysian Medical Council (MMC).
Annual Practising Certificate (APC): Hold a valid APC for the current year.
Completion of a recognized postgraduate qualification in Internal Medicine (e.g., MRCP UK, M.Med (Internal Medicine), or equivalent).
Completion of a specialized Fellowship in Gastroenterology and Hepatology from a recognized training institution/program (e.g., Ministry of Health Malaysia's subspecialty training program, or equivalent overseas fellowship recognized for specialist registration in Malaysia).
National Specialist Register (NSR): Be registered or eligible for registration on the Malaysian National Specialist Register (NSR) as a Specialist in Gastroenterology & Hepatology. This is a mandatory requirement to practice as a specialist in Malaysia.
Must have a valid Professional Indemnity Insurance
Experience: Proven clinical experience in the diagnosis and management of a wide range of liver conditions, typically acquired during fellowship training.
Language Proficiency: Proficient in English and Bahasa Malaysia (Malay) for effective communication with patients and colleagues. Proficiency in other local languages/dialects (e.g., Mandarin, Cantonese) is highly advantageous.
