Job description

Responsible in performing various functions through collaborative drug therapy management and patient care including day-to-day operations and other clinical services related, as well as overseeing Retail Pharmacy and to assist the Head of Pharmacy in the overall management of the Hospital Pharmacy in terms of effective and efficient operation, cost effective supply and safe medication practice.

Job Responsibilities:

Medication Management:
1. Ability to evaluate the appropriateness of medications therapy and dosage accordance to patient's diagnosis.
2. Keep proper documentation of medications plans, patient progress and results of pharmaceutical treatment.
3. Ensure the selection, preparation, and dispensing of medications, including chemotherapy agents, biologics, and supportive care drugs.
4. Responsible for the management and safe handling of radioactive materials and radiopharmaceuticals used in diagnostic imaging and therapeutic procedures.
5. To perform medication dispensing and medication counselling.
6. To obtain and disseminate as necessary drug or drug therapy related information.

Cytotoxic Drug Reconstitution:
7. Review, interpret, and verify oncology prescriptions for accuracy, appropriateness, and adherence to treatment plans.
8. Counter check final CDR product release

Inventory Control/Management:
9. To perform other related duties as assigned and directed by the immediate superior.
10. To be person in charge of scheduling for Pharmacists and Pharmacy Technicians in the service areas.
11. To organize the day-to-day operations in the designated areas of services
12. To ensure that only approved products are stored or available for use in the hospital
13. To ensure the availability of formulary drugs at all service areas
14. To ensure proper inventory management of pharmaceutical products through periodic cycle count at service areas and drug check at all storage locations
15. To ensure the security of dangerous drugs, psychotropic substances and controlled drugs are maintained at all times and its inventory is properly managed.
16. To participate, monitor, assess and review of departmental KPI or quality indicator
17. To undertake any other duties as assigned by the Head of Pharmacy

Research and Clinical Trials:
18. Support oncology research and clinical trials conducted within the hospital by participating in preparation, and administration of investigational drugs.
19. Maintain accurate records and documentation of clinical trial-related activities.
20. To collaborate with medical and healthcare teams to ensure accurate diagnosis and effective treatment of patients.
21. Contribute to a continuous learning environment and innovation to the organization.
22. To supervise clinical activities offered by the department and re-evaluates from to time to time in ensuring that clinical resources are most appropriately used.

Medication Safety and Quality Assurance:
23. To develop clinical pharmacy programs according to policies & procedures.
24. To take reasonable care at workplace for the safety of oneself, others and to comply with any instruction or measurement on occupational safety, health and environment as required.
25. Reports all accidents/ incidents and hazard to the superior and complete the Incident Report Form for each accident/incident.
26. To ensure all documents required by legislations are preserved and stored appropriately

Training & Supervision:
27. To carry out orientation programs for new staff in the department
28. To perform periodic evaluation of respective staffs in the department
29. To develop and implement training programmes for the departmental personnel
30. To participate in the development and implementation of training programs in pharmacy areas for pharmacy staff

Job Requirements:

  • Degree in Pharmacy or its equivalent qualification and registered with Pharmacy Board of Malaysia.
  • Minimum 1-2 years working experience in hospital, preferably in hospital pharmacy.
  • Good communication skills with ability to converse in English.
  • Strong leadership skills and confident in decision making.
  • Good interpersonal skills, customer oriented, dynamic and organized.
