Job summary

A healthcare company located in Penang seeks a dedicated professional to support patients navigating treatment barriers. This role involves working closely with case management and promoting rehabilitation facilities, requiring a background in nursing or social work and experience in customer service. The ideal candidate will possess a commitment to assisting individuals, particularly those with disabilities, and will need to be mobile for patient visits.

Qualifications

  • Minimum of three (3) years' customer service experience, preferably in human services.
  • Experience supporting individuals with disabilities or challenging behaviors is highly preferred.
  • Must be willing to travel and possess own transport.

  • Promote IHT Rehab facilities to patients and families.
  • Assist in tracking patients and data for management reporting.

Job description

Responsibilities

  • Liaise with Case Management unit in PERKESO.
  • Promotes IHT Rehab facilities and service provisions to the patients and family members.
  • Assist in tracking and collating number of patients contacted, home visits and other data as required for reporting to management.
  • Works with patients to identify and eliminate barriers from seeking treatment (i.e. issues of food, housing, transportation); to allow greater participation in treatment as needed.
  • Sets up transportation for patients to attend appointments.
  • Schedules appointments with consideration of patients’ needs and preferences.

Qualifications

  • Diploma/Degree in Nursing, Medical Assistant, Social Work or equivalent.
  • Minimum of three (3) years' customer service experience, preferably in human services, possess demonstrated commitment to customer service.
  • Professional or personal experience supporting individuals with disabilities, or challenging behaviors highly preferred
  • Must be willing to travel and possess own transport.
