  • ≥ 85 % patient adherence to personalized nutrition plans
  • Average improvement of 10 % in key health metrics (e.g., hemoglobin, BMI) within three months
  • 90 %+ patient satisfaction with nutrition counseling
  • Delivery of at least 4 group education workshops per quarter
What will you do
  • Develop individualized plans for prenatal, postnatal, menopause, and chronic-condition care.
  • Conduct one-on-one counseling sessions—assess dietary habits, set goals, and monitor progress.
  • Collaborate with care teams (OB/GYN, endocrinology, nursing) to integrate nutrition into treatment pathways.
  • Lead educational workshops and support groups on topics like breastfeeding nutrition and bone health.
  • Monitor outcomes using KPI dashboards; adjust plans based on lab results and patient feedback.
  • Maintain up-to-date records and document interventions in the patient portal.
How Success is measured
  • Patient adherence rate to nutrition plans
  • Clinical outcome improvements (average % change)
  • Patient satisfaction scores (survey ≥ 90 %)
  • Timely completion of follow-up consultations (> 95 % on schedule)
