Job summary

Ensemble Health Partners India is seeking a Clinical Documentation Senior Specialist to ensure accurate and comprehensive provider documentation supporting severity of illness, risk adjustment, and compliant coding. You will collaborate with physicians, clinical teams and coding professionals, review records for gaps, and drive documentation integrity to optimize reimbursement.

Based in Hyderabad, you will partner with care teams for timely DRG reviews and use CDI software to support data

Collaborates with physicians, clinical teams, and coding professionals to validate documentation integrity and optimize quality reporting and reimbursement. Reviews medical records to identify gaps in clinical documentation and facilitates timely clarification with providers to ensure accurate representation of patient conditions and procedures. Partners with coding and care teams to align documentation with clinical indicators, regulatory requirements, and organizational standards for data integrity and revenue compliance.
