Job summary

CommonSpirit Health at Home is seeking a Registered Nurse for a PRN role serving Leadville, Fairplay and Frisco. In this position you will deliver high‑quality, patient‑centered home health care in patients’ homes, building trusted relationships and guiding care through the full healing process.

You will work with a multidisciplinary team, educate patients and families, and apply the latest clinical documentation tools to maximize patient time and outcomes.

Qualifications

• Graduate of an accredited nursing program with an active RN license.
• Minimum of 1 year of nursing experience; Home Health experience is a significant plus.
• CPR/BLS certification is required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader delivering personalized nursing care in the patient’s home.
• Craft individualized care plans and adjust them as needed.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Educate patients and families, providing essential teaching and emotional support.
• Oversee caseload and mentor other staff to foster a cohesive team.
• Utilize technology to optimize care delivery and maintain accurate documentation.

As a Home Health RN, you will:

Job Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
