Responsibilities

• History taking and examination of patient in day care / in patient department
• Take day care / ward rounds daily before consultant rounds
• Attend rounds with the respective consultants and carry out the necessary changes in the order
• Enter changes in the treatment sheet/HIS and inform the nursing staff on duty
• Follow up with investigations and inform the registrar/consultant
• Attend to patient related clinical issues and manage clinical problems
• Obtain consent for minor procedures like blood transfusion
• Perform minor procedures at ward/day care level like wound management
• Implement clinical management plans as decided by seniors and team
• Raise diagnostic and procedural requisitions and ensure timely completion
• Ensure all medication orders are entered in eMAR/HIS/chemotherapy chart
• Assist in other wards or during shortage or disasters; attend emergencies and inform consultants
• Maintain medical records in HIS including assessments, daily notes and procedure notes
• Attend organization camps/activities as required
• Communicate with consultants, nurses and other clinical departments to provide timely care
• Provide information about patient condition to attendants
• Check diet, medicine, vitals charts regularly
• Coordinate discharge summaries and explain to patient attendants
• Ensure compliance with ISO/NABH accreditation standards and hospital policies

Job description

• Adheres to the departmental policies and procedures, objectives, quality assurance programs, safety, environmental and infection control standards.
• History taking and examination of patient in day care / in patient department.
• Take day care / ward rounds daily before consuJtant rounds.
• Attend rounds with the respective consultants and carry out the necessary changes in the order. Enter them in the treatment sheet/HIS. Inform the nursing staff on duty about the same.
• To follow up with all the investigations to be seen and trace their results. Inform the respective registrar /Consultant.
• Attend to patient related clinical issues and accordingly manage clinical problems
• Obtain consent from patent and attender for minor procedure like blood transfusion.
• Do minor procedures at ward / day care level like Woundmanagement
• Implement clinical management plan for the patient as decided by the seniors and team.
• Raise diagnostic and procedural requisitions as per plan of care and ensure timely completion of the same.
• To ensure all the medication orders are appropriately and timely entered in the eMAR module of HIS/medication chart/ chemotherapy chart.
• To be able to assist in wards / department or wards other than assigned in case of shortage or for disaster management and medical emergencies.
• Attend emergencies and informing consultant doctors for further action.
• To keep all the medical records updated in HIS which includes assessment, daily notes, procedurenotes, medication order.
• To attend camps/activities organized by the organization as and when required.
• To communicate and coordinate with Consultants, nurses, and other clinical departments in providing timely and accurate clinical care to patients
• Providing the information regarding the patients condition to attendant whenever required.
• Check the diet chart, medicine chart, vitals chart frequently
• Co-ordinating for the discharge summary and explaining the same to the patient attender.
• Ensure compliance to ISO/NABH accreditation standards and adhere to hospital policies & procedures.
