Job summary

Day a Rehabilitation Trust in Kerala is seeking a dedicated nurse to join our palliative care team. The role focuses on comprehensive patient-centered nursing, home visits, and collaboration with doctors and allied health professionals.

You will assess needs, manage pain, educate families, maintain records, and ensure safety and infection prevention. This is a patient-focused role with meaningful community impact.

Responsibilities

• Assess patients' physical, emotional, and psychosocial needs and develop individualized care plans.
• Provide nursing care, including medication administration, wound care, catheter care, and symptom management.
• Monitor and manage pain and other distressing symptoms to improve patient comfort and quality of life.
• Assist patients with activities of daily living and promote independence wherever possible.
• Educate patients and family members on disease management, medication use, nutrition, and home care practices.
• Provide emotional support and counseling to patients and caregivers during illness and end-of-life care.
• Coordinate with doctors, therapists, social workers, and other healthcare professionals to ensure comprehensive care.
• Maintain accurate patient records, nursing notes, and care documentation.
• Respond promptly to changes in patient condition and report concerns to the medical team.
• Conduct home visits and follow-up assessments as required by the palliative care program.
• Ensure infection prevention and patient safety standards are maintained at all times.
• Participate in community awareness programs and palliative care initiatives when required.
