Qualifications

  • Graduate of an accredited Nurse Practitioner program.
  • Current, unrestricted Nurse Practitioner/APRN license in good standing.
  • Current national certification as a Nurse Practitioner appropriate to practice area.
  • Current CPR/BLS certification.
  • Eligible for hospital privileges at Pana Community Hospital.
  • Must meet licensure and credentialing requirements to practice as NP.
  • DEA registration as applicable to the position.
  • Ability to obtain Illinois authorization if licensed in another state.

Pana Community Hospital is seeking a skilled, compassionate, and patient-focused Nurse Practitioner (NP) to support our outreach clinics. The NP will provide comprehensive evaluation, diagnosis, treatment, and follow-up care in an outpatient setting while working collaboratively with physicians, nursing staff, and other healthcare professionals.

Responsibilities
  • Evaluate patients presenting for routine, preventive, acute, and chronic care needs in a clinic setting.
  • Obtain and document comprehensive patient histories and perform physical examinations.
  • Assess patient conditions and develop appropriate diagnoses and individualized treatment plans.
  • Order, perform, and interpret appropriate diagnostic testing, including laboratory studies, imaging, and other tests within the Nurse Practitioner's scope of practice.
  • Diagnose and treat patients in accordance with established clinical guidelines, standards of care, applicable laws and regulations, and hospital policies.
  • Prescribe medications and treatments within the Nurse Practitioner's scope of licensure, applicable law, clinical privileges, and established practice requirements.
  • Perform office-based procedures and treatments consistent with the Nurse Practitioner's education, training, competency, scope of practice, and clinical privileges.
  • Educate patients and families regarding diagnoses, treatment plans, medications, follow-up care, and preventive health measures.
  • Recognize patients requiring a higher level of care and facilitate appropriate referral, transfer, or physician consultation when necessary.
  • Provide appropriate follow-up and continuity of care.
  • Maintain accurate, timely, and complete medical records and clinical documentation.
  • Communicate and collaborate effectively with physicians, nursing staff, and other healthcare professionals regarding patient care.
  • Maintain patient confidentiality and comply with HIPAA and organizational privacy policies.
  • Follow established patient safety, infection prevention, quality assurance, regulatory, and risk-management standards.
  • Participate in quality improvement initiatives, clinic activities, meetings, education, training, and competency requirements as assigned.
  • Maintain current knowledge of clinical practices, treatment guidelines, medications, and standards applicable to outpatient clinic practice.
  • Maintain appropriate professional relationships with patients, families, providers, employees, and community members.
  • Travel between outreach clinic locations as required to support patient care.
  • Perform other duties as assigned.
Qualifications
  • Graduate of an accredited Nurse Practitioner program.
  • Current, unrestricted Nurse Practitioner/APRN license in good standing.
  • Current national certification as a Nurse Practitioner appropriate to the individual's area of practice.
  • Current CPR/BLS certification.
  • Eligible for and able to obtain medical staff membership and clinical privileges at Pana Community Hospital.
  • Must meet all applicable state and federal licensure, certification, registration, credentialing, and privileging requirements necessary to practice as a Nurse Practitioner.
  • Must obtain and maintain any state licensure or authorization required to practice at Pana Community Hospital. Candidates licensed in another state must be eligible to obtain the appropriate Illinois authorization prior to providing patient care.
  • Must practice within the scope of applicable federal and state laws and regulations, hospital policies and procedures, medical staff bylaws, clinical privileges, and the Nurse Practitioner's education, training, certification, and demonstrated competency.
  • Must comply with all applicable Illinois requirements governing Nurse Practitioner practice, including requirements related to physician collaboration, practice agreements, delegation, prescriptive authority, or other professional practice requirements, as applicable.
  • Current or eligible DEA registration, as applicable to the position and scope of practice.
  • Successful completion of all required hospital credentialing and privileging processes prior to providing patient care.
  • Successful completion of required background, employment, reference, and credentialing checks.
  • Ability to communicate effectively, both verbally and in writing.
  • Strong clinical assessment, critical-thinking, diagnostic, and decision‑making skills.
  • Ability to work independently within the Nurse Practitioner’s authorized scope of practice while appropriately consulting and collaborating with physicians and other healthcare professionals.
  • Demonstrated commitment to patient‑centered care and service excellence.
Preferred Qualifications
  • Previous experience in family medicine, internal medicine, outpatient clinic, primary care, urgent care, or another ambulatory care setting preferred.
  • Experience evaluating and treating patients across a broad range of ages preferred.
  • Experience with common outpatient procedures and management of acute and chronic conditions preferred.
  • Experience working with electronic medical records preferred.
Schedule

Schedule and hours will vary based on the operational needs of the outreach clinics and may include varying weekday hours. Travel between outreach clinic locations is required.
