The Hans Foundation (THF) invites applications for a Nursing role at the HPHC in Tiruldih, Saraikela, Jharkhand. Responsibilities

Job Summary: The ANM/Staff Nurse will be responsible for providing quality nursing care, maternal and child healthcare, preventive and promotive health services, patient care, health education, and community-level support at the HPHC. The role will work closely with the Medical Officer and other HPHC team members to ensure safe, compassionate, timely, and effective healthcare services to patients and the community.

• Provide quality nursing care to patients as per clinical requirements and instructions of the Medical Officer.
• Record and monitor vital signs, including BP, pulse, temperature, respiratory rate, and SpO , as applicable.
• Assist the Medical Officer during patient consultations, clinical procedures, and emergency care.
• Administer medicines and injections as prescribed and within the scope of professional practice.
• Provide wound care, dressing, basic first aid, and other nursing support as required.
• Monitor patients during and after procedures or treatment and ensure their comfort, dignity, and confidentiality.
• Assist in the stabilization of patients requiring emergency referral and facilitate timely referral to appropriate health facilities.
• Support ANC and PNC services and identify and report signs of high-risk pregnancy for timely intervention and referral.
• Support pregnancy testing, maternal counselling, and provide guidance on nutrition, hygiene, and pregnancy-related danger signs.
• Support newborn and child health activities, including immunization and appropriate child nutrition practices.
• Maintain accurate maternal, child health, and other relevant patient records.
• Provide counselling and health education on family planning methods and reproductive health.
• Support the availability and appropriate distribution of family-planning commodities as per programme guidelines.
• Provide appropriate health information and counselling to adolescent girls and women and facilitate referrals where required.
• Support screening for hypertension, diabetes, anaemia, and other priority health conditions.
• Assist in NCD follow-up, treatment adherence, and identification of high-risk patients in coordination with the Medical Officer.
• Conduct and support health-awareness and community education sessions on nutrition, hygiene, sanitation, preventive healthcare, and healthy lifestyle practices.
• Participate in village-level outreach activities, community screening programmes, and health camps.
• Coordinate with ASHA workers and other frontline health workers to support community-level healthcare activities.
• Mobilize and guide community members to access HPHC services and support follow-up of high-risk and vulnerable patients.
• Follow standard infection prevention and control practices, including proper hand hygiene and use of PPE as required.
• Ensure safe handling, segregation, and disposal of biomedical waste in accordance with applicable guidelines.
• Maintain cleanliness and hygiene of the nursing and clinical areas.
• Follow patient-safety protocols during clinical procedures and medication administration and immediately report any adverse events, injuries, or safety concerns.
• Identify patients requiring higher-level care in coordination with the Medical Officer and facilitate timely referrals.
• Provide patients and attendants with appropriate information and guidance regarding referrals and follow-up care.
• Maintain accurate referral records and support follow-up of referred patients.
• Maintain nursing records, registers, and other required documentation accurately and on time.
• Record patient vitals, procedures performed, medicines administered, and other relevant clinical information.
• Maintain ANC/PNC, NCD, referral, health-camp, and other programme-related records as applicable.
• Ensure timely and accurate data entry in applicable digital and physical systems.
• Support the preparation and submission of daily, monthly, and other programme reports as required.
• Maintain strict confidentiality of patient and programme-related information.
• Perform any other duties related to nursing, patient care, community health, and HPHC operations as assigned by the Medical Officer or supervisor.
• Certification is must for the role
