Job Description
We are seeking a skilled and tech-savvy Medical Scribe who is eager to work with emerging technologies and support healthcare providers by improving efficiency and reducing administrative workload. The ideal candidate should have hands‑on experience with EHR/EMR systems and the ability to accurately document patient encounters in real time while adapting to different provider styles and workflows.
Location: Hyderabad, India (HITEC City) – Work from Office
Key Responsibilities
- Review physician dictations and transcribe them into accurate clinical notes within EMR/Billing systems.
- Prepare and assemble medical record documentation and patient charts for physicians.
- Ensure medical record compliance through proper documentation attestation.
- Update patient history, physical examination findings, and other relevant health information in the patient chart.
- Prepare and send documentation to physicians for review and approval in accordance with facility-specific procedures.
- Monitor basic lab results and screening procedures.
- Comply with hospital and medical facility policies, including HIPAA and Joint Commission standards.
- Perform clerical and administrative tasks to improve provider productivity and clinic workflow.
- Assign diagnosis and procedure codes using ICD and CPT coding systems.
- Ensure coding accuracy and proper sequencing in compliance with government and insurance regulations.
- Follow up with providers regarding insufficient or unclear documentation.
- Communicate with clinical staff regarding documentation and coding requirements.
- Research and resolve complex or unusual coding cases.
- Review patient charts and documents for completeness and accuracy.
- Review previous-day patient notes for evaluation and coding.
- Ensure all coding references and standards remain current and active.
Required Skills & Qualifications
- 3–5 years of experience as a Medical Scribe.
- Hands‑on experience with EHR/EMR systems.
- Strong typing and documentation skills.
- Proficiency in Microsoft Office Suite and Microsoft 365.
- Strong administrative writing and reporting skills.
- Excellent organizational and record‑keeping abilities.
- High level of professionalism, confidentiality, and attention to detail.
- Strong verbal and written communication skills.
Education & Certification Requirements
- Certification in Medical Coding or successful completion of a coding certification program.
- Bachelor’s degree in a pre‑healthcare or related field preferred.
- Strong knowledge of anatomy, physiology, and medical terminology.
- Familiarity with ICD coding systems and CPT procedures.
- Experience using medical record software, database systems, and word‑processing tools.
