Job summary

Graphic Era Dehradun seeks a Medical Records professional to join its MRD team. You will review patient records, ensure completeness, authenticate documents, and support NABH/NMC audits to uphold compliance. The role emphasizes accuracy, confidentiality, and cross-department coordination.

The ideal candidate holds a BAMS with valid registration and at least one year of clinical experience, ready to liaise with doctors, nursing staff, and administration to maintain flawless medical records.

Qualifications

• BAMS degree with valid registration under the respective state system.
• Minimum 1 year of relevant clinical experience required.
• Experience with NABH/NMC guidelines and medical records workflows preferred.

Responsibilities

• Review and verify patient medical records for completeness and accuracy.
• Ensure documentation, diagnoses, treatment details, and discharge summaries are complete.
• Conduct medical record audits and identify gaps.
• Coordinate with doctors, nursing, and clinical departments to finalize records.
• Assist in maintaining medical records per hospital policies and NABH/NMC requirements.
• Support MRD statistics, MIS reports, and related documentation.
• Ensure proper filing, indexing, retrieval, and confidentiality of records.
• Assist during internal, NABH, NMC, and other regulatory audits.
• Support medico-legal documentation and ensure records follow guidelines.
• Coordinate with Billing, TPA/Insurance, Quality for MRD requirements.
• Perform other MRD duties as assigned.

Qualification

• BAMS (Bachelor of Ayurvedic Medicine and Surgery) from a recognized institution/university.
• Valid registration with the respective State/Indian system of medicine council, as applicable.

Experience

• Minimum 1 year of relevant clinical experience required.
• Experience in community healthcare, primary healthcare, rural healthcare will be preferred.

Key Responsibilities

• Review and verify patient medical records for completeness, accuracy, and proper documentation.
• Ensure clinical records, diagnosis, treatment details, discharge summaries, and other documents are complete and properly authenticated.
• Conduct medical record audits and identify documentation gaps or discrepancies.
• Coordinate with doctors, nursing, and clinical departments for completion and correction of medical records.
• Assist in maintaining medical records as per hospital policies and NABH/NMC requirements.
• Support preparation of MRD statistics, MIS reports, and clinical documentation-related reports.
• Ensure proper filing, indexing, retrieval, and confidentiality of patient medical records.
• Assist during internal, NABH, NMC, and other regulatory audits.
• Support medico-legal documentation and ensure records are maintained as per applicable guidelines.
• Coordinate with Billing, TPA/Insurance, Quality, and other departments for medical record-related requirements.
• Perform other duties related to MRD as assigned by the reporting manager.
