Job summary

Frontpoint Health is seeking a skilled Hospice LVN in Stafford, TX to provide compassionate in-home nursing care under the direction of a registered nurse or physician. The role requires a current Texas LVN license, at least one year of hospice experience, and strong communication and teamwork skills to support patients and families in a home-care setting.

You will assess health status, administer medications, monitor vitals, document in EMR, and participate in interdisciplinary team meetings

Qualifications

• Current LVN license in the state of Texas.
• Minimum of one year of hospice care experience.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong critical thinking and problem-solving skills.
• Valid driver's license and reliable transportation.
• Ability to pass a background check and drug screening.
• Wound care 2-3 x week.
