Position and location: Social Worker, Department: Child & Adolescent Outpatient Program, Location: Off Hospital Sites (Other sites employee may work at: Oakville). Status: Permanent Full Time. Hours: Monday to Friday – Day shift and 1 evening per week (hours subject to change based on operational requirements).

Responsibilities
  • To provide services for children, adolescents and transitional aged youth with a mental illness disorder.
  • Conduct a comprehensive assessment and develop a treatment plan.
  • Employ a range of evidenced-based therapies designed to improve outcomes for clients.
  • Engage in case conferences/consultations with treatment providers, including physicians, schools and other community organizations as deemed appropriate, to advocate on behalf of the client.
  • Maintain current information on community resources and linkages with relevant social service and other agencies.
  • Conduct/co-facilitate CBT, DBT groups for Children and Adolescents.
  • Provide short and long-term therapy and case management based on client needs.
Qualifications
  • Master of Social Work (child focus) required
  • Must have membership in OCSWSSW
  • Minimum 3-5 years of prior clinical experience in child and adolescent mental health with holistic assessment, treatment and evaluation of clients
  • Strong clinical skills – assessment, counseling, and discharge planning/community integration
  • Knowledge/skills in CBT, DBT and Solution Focused Therapy preferred
  • Strong Family Therapist background
  • Knowledge and skills related to crisis intervention
  • Previous group experience with this population
  • Commitment to professional development and ability to document according to college standards
  • Ability to function independently and think critically to problem solve
  • Working knowledge of current best practices for addictions treatment
  • Capacity to deliver culturally sensitive and appropriate patient care
  • Commitment to customer service and teamwork
  • Knowledge of Halton Community Resources an asset
  • Excellent employment and attendance record
  • Strong communication and collaborative skills
  • Knowledge of DSM as it relates to child and adolescent population
  • Previous experience with electronic documentation preferred
