Responsibilities

  • Assess patients before, during, and after chemotherapy administration.
  • Review medical history, allergies, laboratory investigations, and treatment orders.
  • Evaluate vital signs, weight, pain score, and performance status.
  • Identify contraindications for chemotherapy administration and elevate concerns to the oncologist.
  • Verify chemotherapy orders using the Five Rights of medication administration.
  • Administer chemotherapy, immunotherapy, targeted therapy, blood products, and supportive medications as per hospital protocols.
  • Ensure safe handling and administration of cytotoxic drugs.
  • Maintain appropriate infusion rates and monitor infusion pumps.
  • Follow double-check procedures before administering chemotherapy.
  • Maintain strict aseptic techniques.
  • Follow standard precautions and transmission-based precautions.
  • Ensure proper use of PPE while handling cytotoxic medications.
  • Prevent occupational exposure to hazardous drugs.
  • Report needle-stick injuries and exposure incidents promptly.
  • Maintain accurate nursing records.
  • Record chemotherapy administration details.
  • Document adverse events, medication errors, and incident reports.
  • Complete chemotherapy consent verification.
  • Ensure traceability of chemotherapy medications.
  • Follow NABH standards and hospital quality indicators.
  • Participate in quality improvement initiatives.
  • Report medication errors and near misses.
  • Ensure compliance with patient safety goals.
  • Participate in departmental audits.

1. Patient Assessment
  • Assess patients before, during, and after chemotherapy administration.
  • Review medical history, allergies, laboratory investigations, and treatment orders.

  • Identify contraindications for chemotherapy administration and elevate concerns to the oncologist.
2. Chemotherapy Administration
  • Verify chemotherapy orders using the "Five Rights" of medication administration.
  • Administer chemotherapy, immunotherapy, targeted therapy, blood products, and supportive medications as per hospital protocols.
  • Ensure safe handling and administration of cytotoxic drugs.

3. Patient Monitoring
  • Continuously monitor patients for adverse drug reactions and infusion-related complications.
  • Identify early signs of hypersensitivity reactions, extravasation, and chemotherapy toxicity.
  • Initiate emergency management as per protocol and inform the oncologist immediately.
  • Document all observations and interventions accurately.
4. Infection Prevention & Safety
  • Maintain strict aseptic techniques.
