Job Description
We are seeking skilled and compassionate Care Coordinators with 2-3 years of experience in healthcare or care management. The Care Coordinator will assist in organizing and managing care for patients, ensuring that they receive the appropriate services, treatments, and support. This role involves collaborating with multidisciplinary teams, monitoring patient care progress, and advocating for patients to navigate complex healthcare systems. The ideal candidate will be detail-oriented, organized, and dedicated to enhancing patient experiences and outcomes.
- Care Coordination and Patient Advocacy: Coordinate and oversee care plans for patients, ensuring that all aspects of care are delivered effectively and in a timely manner. Act as the primary point of contact for patients, caregivers, and healthcare providers, ensuring that the patient’s needs are communicated and addressed. Advocate for patients by identifying barriers to care, such as financial, transportation, or social issues, and working to resolve them.
- Scheduling and Appointment Management: Schedule patient appointments, tests, treatments, and procedures based on care plans and patient needs. Follow up with patients to ensure they attend scheduled appointments, making rescheduling arrangements when necessary. Monitor and manage care timelines to ensure that patients receive timely and necessary services.
- Collaboration with Healthcare Providers: Work closely with physicians, nurses, social workers, and other healthcare professionals to ensure a comprehensive and coordinated approach to patient care. Facilitate communication between patients and providers to ensure that all aspects of the care plan are executed. Collaborate with specialists, home health agencies, and community resources to provide holistic care and ensure continuity of care.
- Patient Education and Support: Educate patients and their families about treatment options, medications, preventive care, and self-care strategies. Provide resources and information to help patients manage their conditions, including connecting them to support groups or community services as needed. Help patients understand their rights and responsibilities within the healthcare system.
- Monitoring and Follow-Up: Track patient progress and monitor health outcomes to ensure that care plans are being followed and that the patient is improving. Follow up with patients after hospitalizations, surgeries, or significant treatments to ensure they are recovering as planned and that they understand any ongoing care requirements. Ensure proper documentation and follow-up on any missed appointments or non-compliance with care instructions.
- Documentation and Reporting: Maintain accurate, up-to-date records of patient interactions, care plans, appointments, and progress. Document any issues or concerns in the patient’s file and escalate them to appropriate team members for resolution. Prepare regular reports on patient care progress and outcomes for internal and external stakeholders as needed.
- Quality Improvement: Identify opportunities for improving the care coordination process, optimizing resource utilization, and enhancing patient satisfaction. Participate in quality improvement initiatives aimed at improving patient care, patient flow, and care team communication. Contribute to the development and implementation of protocols and processes that improve care coordination outcomes.
- Compliance with Regulations: Ensure that all care coordination activities are compliant with healthcare regulations, including HIPAA, and organizational policies. Maintain confidentiality of patient information and ensure that all documentation is managed according to legal and ethical standards.
Requirements
- 2-3 years of experience in healthcare settings, preferably in care coordination, case management, or a related field.
- Experience working with patients in a care management or coordination role, including scheduling, referrals, and follow-up.
- Familiarity with medical terminology, healthcare systems, and patient care processes.
- Communication Skills: Strong verbal and written communication skills for interacting with patients, healthcare providers, and internal teams.
- Organizational Skills: Ability to manage multiple priorities, including patient appointments, care plans, and follow-up activities.
- Problem-Solving: Ability to address patient care barriers and find creative solutions to healthcare challenges.
- Attention to Detail: Strong attention to detail in managing patient information, documentation, and care schedules.
