Overview

Social Worker, Palliative Care, Oc. Kildare

Cpl Healthcare, in partnership with the HSE are recruiting for an experienced Social Worker to work in a specialist palliative care service based in Co. Kildare.

You will work as an integral part of a multidisciplinary team, based in a 16 bedded inpatient unit which provides symptom management, pain control, respite & End of life Care and will provide psychosocial support, guidance & follow up contact.

You will also provide a Social Work Service to clients in the community linked in with the Palliative Home Care Team in a sensitive and holistic way aimed at improving the quality of life for people living with life limiting conditions and to provide comfort and dignity at end of life.

Responsibilities
  • Provide a medical social work service for clients of the specialist palliative care service, including assessment, crisis intervention, counselling, family work, discharge planning with other team members, and helping patients and families access health, social and welfare entitlements in the community.
  • Work closely with other team members in responding to the social and emotional impact of terminal illness and hospitalisation on patients and their families.
  • Bereavement follow up where appropriate.
