Job summary

Horizons - Cope Foundation in Cork Communities is seeking a Social Care Worker for An Cuan, a Regional support service for people with intellectual disabilities. The role involves implementing PCP and PBS plans and working with an interdisciplinary team to improve quality of life.

The successful candidate will collaborate with PBSS, support PCP documentation, and act as a liaison for external stakeholders across multiple locations. A 2-year fixed contract is offered, aligned with HSE scales.

Qualifications

  • A minimum of three years’ experience of working with people who have an intellectual disability/autism and behaviours that challenge.
  • A relevant third level qualification in Social Care or equivalent as per CORU requirements.
  • A working knowledge of Person-Centred Planning, Positive Behaviour Support and positive risk.

Responsibilities

  • Work collaboratively with the Positive Behaviour Support Specialists for PBS data assessment, planning, implementation, review and documentation.
  • Support the development of Person-Centred Plans and documentation associated with same.
  • Be the point of contact for external stakeholders.
  • Travel to various locations to carry out work.
  • Partake in the delivery of PCP and PBS presentations with PBS specialists.
  • Liaise with other members of the An Cuan IDT when required and appropriate.

Description

An Cuan is a regional support service for people with an intellectual disability whose quality of life is compromised as a result of them communicating through behaviours that challenge.

An Cuan assists individuals and their circle of support (including services and families) to enhance their quality of life through the implementation of an effective Person-C c entred Plan (PCP), Positive Behaviour Support (PBS) plan, and through the provision of interdisciplinary recommendations.

This position is an exciting and unique opportunity for a Social Care Work professional to work as part of an interdisciplinary team (IDT) to develop their knowledge, skills and interest in the areas of PCP and PBS.

Social Care Worker x 1 (2-year fixed contract)

Aligned to the HSE Salary scale for Social Care Worker as of the 01/06/2026:

New appointees to any grade start at the minimum point of the scale. Incremental credit will be applied for recognised relevant service in Ireland (Department of Health Circular 2/2011). Incremental credit is normally granted on appointment, in respect of previous experience in the Civil Service, Local Authorities, Health Service and other Public Service Bodies and Statutory Agencies.

The successful candidate will:

  • Work collaboratively with the Positive Behaviour Support Specialists (PBSS) for PBS data assessment, planning, implementation, review and documentation.
  • Support the development of Person-Centred Plans and documentation associated with same.
  • Be the point of contact for external stakeholders.
  • Travel to various locations (dependent on the needs of the person being supported and their circle of support) to carry out their work.
  • Partake in the delivery of PCP and PBS presentations that are delivered with the PBS specialists.
  • Liaise with other members of the An Cuan IDT when required and appropriate.

Applicants must possess the following:

  • A minimum of three years’ experience of working with people who have an intellectual disability/autism and behaviours that challenge.
  • A relevant third level qualification in Social Care or equivalent as per CORU requirements.
  • A working knowledge of the implementation of Person-Centred Planning, Positive Behaviour Support and positive risk.
  • Confidence and self-motivation, excellent interpersonal skills, planning and organisational skills with the ability to build rapport, negotiate and liaise effectively with others (i.e. with families/other members of the interdisciplinary team).
  • The ability to problem solve and facilitate problem solving.
  • A flexible and creative approach to service delivery.
  • Very good IT skills.
  • A full clean driving licence and their own vehicle.
