Bridhaven Nursing Home in Mallow is recruiting a Social Care Lead to design and lead an innovative, person-centred activity programme for residents. Qualifications

• Degree in Social Care or relevant field.
• Experience with older persons preferred.
• Highly efficient and organized.
• Excellent interpersonal skills.

Responsibilities

• Lead the planning, implementation and evaluation of person-centred activity programmes for Bridhaven residents.
• Schedule social sessions to maximize benefit for residents and care team.
• Encourage resident participation while maintaining independence and dignity.
• Consult with residents to identify social and recreational needs and preferences.
• Produce a monthly activities programme and communicate it to residents.
• Collaborate with multidisciplinary team to feed back on resident status at care plan reviews.
• Build relationships with residents based on respect and equality to promote independence.
