Job Description:

TerraGlen is devoted to working in partnership with young people, their families, representatives, and others to deliver a specialist, person-centred, and outcome-focused service for individuals with disabilities.

Responsibilities:
  • Motivate, support, and encourage service users to engage in planned activities of their choice.
  • Support the care of the service user, including personal care and providing a general overview of their daily needs.
  • Support and promote independence and community integration in line with New Directions (HSE, 2012).
  • Educate yourself about each service user, listen to them, and use reflective practice to enhance your skills and knowledge.
  • Participate in a key worker role under guidance from a senior staff member.
  • Work as part of a team, supporting colleagues and attending regular meetings and supervisions.
  • Maintain confidentiality in accordance with GDPR.
  • Manage medication in line with policy.
  • Liaise with multidisciplinary teams, families, and relevant persons regarding the service user.
  • Observe and report on health and safety matters.
  • Contribute to person-centred planning meetings and follow up on actions and outcomes.
  • Support service users in daily life, helping them make appropriate choices.
  • Report concerns about welfare, safety, or health.
  • Record accurately per national standards and policies.
  • Support management of behaviours of concern using positive behaviour support plans.
  • Participate in audits and ensure follow-up actions.
  • Follow financial procedures and report discrepancies.
  • Build positive relationships with families and relevant contacts.
  • Develop and maintain professional rapport with colleagues.
  • Create opportunities for community involvement, volunteering, employment, or education for service users.
  • Attend all mandatory training and development programs.
  • Engage in supervision and appraisal, reflecting on practice to improve skills.
  • Perform other relevant duties within the scope of the social care worker role.
Qualifications:
  • Minimum Level 7 in Applied Social Care or a relevant qualification.
  • Full driver's license with two years' experience (preferred).
  • Currently undertaking or holding a CORU-recognised Social Care Qualification.
