Job summary

Medforce is seeking an experienced Occupational Therapist to deliver person-centred assessment, intervention, and rehabilitation in a primary care setting across clinic, home, and community visits. The role supports MDT working, promotes self-management, and aims to maximise functional independence for clients with diverse needs, including older adults and chronic conditions.

Responsibilities

• Conduct comprehensive occupational therapy assessments in clinic, home, community, or virtual settings.
• Develop and implement individualised intervention plans based on client goals and needs.
• Assess ADLs, mobility, cognition, sensory processing, and environmental factors.
• Provide rehabilitation and reablement interventions to maximise independence.
• Recommend and prescribe adaptive equipment, assistive technology, and environmental modifications.
• Deliver falls prevention, fatigue management, energy conservation, and self-management programmes.
• Support clients with chronic conditions and age-related decline.
• Collaborate with families, carers, and support networks.

Occupational Therapist – Primary Care

The Occupational Therapist (OT) will provide person-centred occupational therapy assessment, intervention, rehabilitation, and health promotion services within a primary care setting. This role will be open to both Staff with experience or Senior OTs.

The Occupational Therapist will deliver evidence-based interventions aimed at preventing hospital admission, facilitating early discharge, promoting self-management, and supporting functional independence.

Responsibilities of Occupational Therapist – Primary Care

• Conduct comprehensive occupational therapy assessments in clinic, home, community, or virtual settings.
• Develop and implement individualised intervention plans based on client goals and occupational needs.
• Assess functional ability relating to activities of daily living (ADLs), mobility, cognition, sensory processing, and environmental factors.
• Provide rehabilitation and reablement interventions to maximise independence.
• Recommend and prescribe adaptive equipment, assistive technology, and environmental modifications where appropriate.
• Deliver falls prevention, fatigue management, energy conservation, and self-management programmes.
• Support clients with chronic conditions, physical disabilities, neurological conditions, mental health needs, and age-related functional decline.
• Work collaboratively with families, carers, and support networks.
• Maintain accurate, timely, and legally compliant clinical documentation.
• Participate in discharge planning and onward referrals as required.

Multidisciplinary Team Working

• Work as an integral member of the Primary Care Multidisciplinary Team (MDT).
• Liaise effectively with:
• General Practitioners (GPs)
• Physiotherapists
• Speech & Language Therapists
• Public Health Nurses
• Social Workers
• Dietitians
• Psychologists
• Community Support Services
• Participate in MDT meetings, case conferences, and care planning discussions.
• Contribute to integrated care pathways and community-based service development.

Health Promotion & Prevention

• Promote healthy ageing and independent living.
• Deliver education and advice to clients, carers, and community groups.
• Contribute to preventative care initiatives within the primary care setting.
• Identify risks that may impact safety, independence, or wellbeing.

Governance & Quality

• Adhere to professional standards, policies, procedures, and legislation.
• Maintain registration and professional competence requirements.
• Participate in clinical audit, service evaluation, and quality improvement initiatives.
• Ensure compliance with safeguarding and risk management procedures.
• Engage in continuing professional development (CPD) and reflective practice.
• Support student supervision and mentoring where appropriate.

Requirements of Occupational Therapist – Primary Care

• Degree or recognised qualification in Occupational Therapy.
• Current registration with CORU

• Experience working as an Occupational Therapist in community, rehabilitation, primary care, or related settings.
• Experience conducting functional and environmental assessments.
• Experience working within multidisciplinary teams.
• Experience managing a varied caseload independently.
