Job summary

CareChoice is seeking a committed Medical Social Worker for The Marlay Nursing Home in a full-time capacity. You will provide comprehensive social work services to residents and families, including assessments, planning, and coordination with the multidisciplinary team to meet care needs.

The ideal candidate is registered with CORU and will maintain annual registration, contributing to high-quality care and advocacy within a supportive team environment at CareChoice.

Qualifications

• Be registered on the Social Workers Register maintained by CORU.
• Provide proof of Statutory Registration on the Social Workers Register maintained by CORU.
• On appointment, maintain annual registration on the Social Workers Register maintained by CORU.

Responsibilities

• Deliver a quality social work service to residents and families, including psycho-social assessment, intervention, and discharge planning.
• Work with the multi-disciplinary team on assessment outcomes and care planning.
• Advise and collaborate with residents, families, and external services to meet needs.
• Attend multidisciplinary team meetings and case conferences as required.
• Support residents to advocate for their own needs or advocate on their behalf.
• Develop relationships with residents and statutory/non-statutory agencies to coordinate care.

Key Responsibilities

• Deliver a quality service to service users ensuring professional standards are maintained in accordance with the requirements as set out by CORU
• Work within current legislation, relevant policies and procedures, guidelines and protocols as laid down by the employer.
• Incorporate Social Work values and ethical principles in planning, developing, implementing and reviewing interventions.
• Provide a comprehensive social work service to residents and families, to include psycho-social assessment, intervention, discharge planning and follow up or referral on, as required, to statutory and / or voluntary services.
• Make it possible for residents to advocate for their own needs, or where appropriate advocate on behalf of service users.
• Work with and advise the multi-disciplinary team in the relevant areas on assessment outcomes.
• Deliver social work service in collaboration with other disciplines / agencies as required, in appropriate settings reflecting the needs of the service user.
• Develop effective working relationships with and between residents, statutory and non-statutory agencies and professionals.
• Attend multidisciplinary team meetings and case conferences as required.
• Actively participate as a member of the relevant service in team building and change management initiatives.
• Keep Line Manager fully informed and up-to-date on all significant matters.

The ideal candidate will:

• On appointment practitioners must maintain annual registration on the Social Workers Register maintained by the Social Workers Registration Board at CORU.
