Job description

We are looking for a compassionate LPN to join our growing team! The right candidate has a can-do attitude, excellent interpersonal skills, and a current state LPN license. In this role, you will provide medical support to our clients while ensuring a safe environment for them.

Responsibilities

1. Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.
2. Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.
3. Prepare clinical/progress notes and summaries.
4. Assist the patient with activities of daily living while teaching appropriate self-care techniques.
5. Provide and maintain a safe environment for the patient.
6. Assist the physician and Registered Nurse in complex nursing situations.
7. Follow universal precautions whenever providing patient care.
8. Follow emergency procedures in case of catastrophic events, incidents, accidents, injuries, or significant changes in the patient's condition.

Qualifications

1. Minimum of 1 year experience as an LPN in a home care setting.
2. Current state license as an LPN.
3. Graduate of an accredited practical nursing program.
4. Proof of current CPR certification.
5. Ability to work a flexible schedule and travel locally.
6. Excellent interpersonal skills.
7. Valid driver’s license and reliable transportation.
