Responsibilities

• Assess, plan, implement and evaluate patient care within professional standards.
• Collaborate with the multi-disciplinary team to coordinate care and discharge planning.
• Maintain accurate written records of patient care and outcomes.
• Support HIQA standards and hospital protocols in daily practice.
