Job summary

Focus Ireland is seeking a Project Worker to join the Aftercare team in Clare, delivering flexible housing support for young people transitioning to independent living. You will implement care plans, provide holistic support, and advocate for residents within a multi-disciplinary framework.

The role requires a degree in social care or related field and CORU registration or eligibility, plus experience in key-working and managing challenging behaviour.

Qualifications

• Degree or higher in social care, youth work, social studies, psychology or related field.
• CORU registration as a Social Care Worker or eligibility.
• Experience delivering person-centred support to young people.

Responsibilities

• Work within Focus Ireland's framework and care model to support residents.
• Deliver high-quality aftercare housing services within a team.
• Develop and implement individual care plans for independent living.
• Provide holistic support including self-care, health, budgeting and domestic skills.
• Advocate for residents regarding housing, welfare, education and training.
• Maintain accurate case records and participate in supervision and reviews.
• Liaise with external agencies and participate in multi-agency work.
