Job description

Senior Physiotherapist

The Senior Physiotherapist will play a key role in delivering timely and effective rehabilitation interventions as part of the Early Supported Discharge (ESD) ICPOP Team based in Naas. Working within a multidisciplinary team, the post holder will support older persons transitioning from acute hospital care to home, facilitating functional recovery, reablement, and optimal independence.

Responsibilities of Senior Physiotherapist

  • Deliver comprehensive physiotherapy assessments and evidence-based interventions to older adults in a community and home-based setting.
  • Contribute to safe and efficient discharge planning in collaboration with acute hospitals and community services.
  • Work closely with MDT colleagues including Occupational Therapists, Nurses, Medical Staff, Social Workers, and Case Managers.
  • Identify risks and implement strategies to minimize re-admissions and falls.
  • Monitor and review patient outcomes using standardized assessment tools.
  • Lead on care planning and documentation, ensuring high standards of clinical governance.
  • Provide supervision and mentoring to basic grade physiotherapists and students.
  • Contribute to the development of protocols, service improvement initiatives, and performance audits.

Requirements of Senior Physiotherapist

  • CORU registration as a Physiotherapist.
  • Minimum 3 years post-graduate clinical experience, with at least 1 year in an older persons or rehabilitation setting desirable
  • Full clean driving licence and access to a car.

Desirable:

  • Experience in ICPOP, ESD models, or integrated care settings.
  • Postgraduate training in areas such as falls prevention, neurological rehabilitation, or geriatrics.
