Job summary

HSE Dublin and North East is seeking a Network Rehabilitation Coordinator to act as the single entry for referrals into the MCRN and direct individuals to appropriate services in the community and hospital network.

The role requires coordinating referrals, conducting assessments of rehabilitation needs, and ensuring documentation supports an integrated post-acute pathway. Autonomy and collaboration within an interdisciplinary team are essential.

Responsibilities

• Facilitate decision making regarding referrals to the MCRN
• Assessment of rehabilitation need
• Introduction of standardised assessment of need
• Coordinate onward referral and seamless transfers between post-acute rehabilitation services within the MCRN
• Responsible for ensuring the completion of relevant rehabilitation documentation, to assist with the implementation of seamless onward post-acute rehabilitation pathway
• Provide clinical leadership to develop and support all staff involved in referring to post-acute rehabilitation
