This full-time permanent role leads patient flow, collaborates with the MDT, and ensures care is holistic, efficient and aligned with best practice within a major acute hospital. The CNM2 will drive discharge planning, staff development, audits, KPI achievement, and inter-disciplinary collaboration, while supporting HR and hospital policy adherence.

Responsibilities

• Assists patient flow as CNM2 to ensure holistic, efficient care
• Engages patients in decision making and collaborates with MDT and staff
• Manages patient flow within and across hospital departments
• Collaborates with consultants and AHP teams to deliver evidence-based care
• Oversees discharge planning and education with ward managers
• Participates in MDT meetings and supports staff development and retention
• Ensures compliance with health and safety, fire and statutory regulations
• Supports HR processes and adherence to hospital policies
• Contributes to audits of nursing standards, care planning and manpower
• Leads and participates in projects within the division and hospital
• Attends division meetings and CNM meetings to achieve patient flow KPIs
• Maintains KPI targets and updates risk registers monthly
• Engages MDT in service provision and development like discharge hubs
• Promotes interdisciplinary care and organisational goals
• Ensures CPD compliance within the division
• Participates in interview panels as required
• Works with HR and other departments on assigned tasks
• Involves with interview panels as required
• Supports the Candidate Information Booklet process and related communications
