Responsibilities

• Act as a Key Worker, supporting development and review of support plans.
• Promote independence and autonomy for service users.
• Assist with daily living activities including personal care.
• Support community inclusion and participation in activities.
• Communicate effectively in an accessible manner, using low arousal approaches.
• Maintain records and participate in training and reflective practice.
• Support safeguarding, health, safety and wellbeing per policy.
