Job summary

Qualifications

• A full QQI level 5 Health Care Qualification is essential.
• Experience in supporting people with complex needs is desirable.
• Ability to work on own initiative within a person-centred framework.

Responsibilities

• Provide person-centred support to individuals with complex needs.
• Contribute to Programme Planning and development of Person Centred Plans.
• Support engagement and communication with service users and families.
