Job summary

Summit Care Inc. is seeking a Physical Therapist Assistant to provide patient care under the supervision of a Physical Therapist. You will perform assessments, implement treatment plans, and deliver home programs for adult and geriatric patients in a facility setting.

The PTA will document services, participate in family and team conferences, manage equipment and supplies, and ensure compliance with state practice guidelines and facility standards.

Qualifications

• Successful completion of an accredited Physical Therapist Assistant Program.
• Valid Physical Therapist Assistant License to practice in appropriate state(s).
• Effective written and verbal communication skills.

Responsibilities

• Provides therapeutic treatment in accordance with the established plan of care.
• Documents treatment provided including reasons for skilled intervention, observation, recording and reporting patient status and response to treatment.
• Participates in the maintenance of therapy records.
