What you will do:

• Collaborate with CAMHS, children’s services, and adult mental health teams to create and implement smooth transition plans.
• Advocate for young people’s needs by developing personalised care plans to promote their independence and well-being.
• Engage with families and carers to support their involvement in the transition process.
• Conduct comprehensive assessments to identify support needs and help develop community engagement and independent living skills.
• Work within Care Act principles to ensure young people can gain employment and live independently.
• Manage a varied caseload, maintaining up-to-date records of interventions and outcomes.
