Job summary

Horizon Care Ltd in Portsmouth is inviting applications for a Youth Support Worker to help young people transition to independent living. You will provide emotional and practical support, develop tailored plans, and collaborate with education and local services.

The role involves safeguarding, risk assessment, and flexible shifts including evenings, weekends and sleep-ins. You will work within a supportive team in a leading provider of residential care and education for young people in the UK.

Qualifications

• Experience working with vulnerable young people.
• Strong safeguarding knowledge and assessment skills.
• Empathetic and resilient when working with young people.
• Excellent communication and problem-solving abilities.
• Willingness to work evenings, weekends and sleep-ins.

Responsibilities

As a Youth Support Worker you will be helping young people transition to independent living, duties include:

• Provide emotional and practical support, including out-of-hours on-call.
• Develop and review support plans, ensuring safeguarding and risk management.
• Help young people access education, training, and employment.
• Manage properties and liaise with other professionals.
• Support our young people to develop independent living skills to prepare them to live fully independently.
